Student Athlete Information
First-year Student Athletes
All first-year Wake Forest University student athletes (freshman or tranfers) must submit the following:
1. Initial Health History Form
2. Student Athlete Insurance Form
3. Copy of Health Insurance Card (front and back)
4. FERPA Student Athlete Waiver Form
5. NCAA Release of Information Form
6. Sickle Cell Form
7. Release of Steudent Health Records Form
Returning Student Athletes
All returning Wake Forest University student athletes must submit:
1. Annual Health History Form
2. Student Athlete Insurance Form
3. Copy of Health Insurance Card (front and back)
4. Sickle Cell Form
