University Police Forms

The Wake Forest University Police Department is concerned about your safety on campus. We are always working to help make your experience on our campus enjoyable and crime-free. Bicycle Registration
The University Police strongly recommends that all bicycles on campus be registered with them.

Event Application Request
Complete this form to have an "Event Packet" mailed to you.

Operation ID
Register your personal property with the University Police to discourage theft.

Demonstration/Protest Policy and Permit
Read the University Policy for Public Gatherings and print Application Form.

Online Traffic Appeal or
Traffic Appeal Form
To appeal a traffic citation, either complete the online appeal or print and complete the Traffic Appeal Form and return to the Department of Parking Management to be forwarded to the Traffic Appeals officer. Detail instructions of the appeals process are on the second page of the form.
