The Repertory Hour Request form may be filled out in Microsoft Word and printed. Jury and Audition Application forms must be completed and emailed to Individual Instructor for approval. The Music Lab Access form is available only as a .PDF. Please print, fill out and return to Jolie Tingen, Instructional Technology Consultant.

Individual Instruction Scholarship Application Form
For both semesters of the 2011-2012 academic year. Awards will be made for the Fall 2011 semester, then applicants will be notified if their scholarship will be continued into the Spring 2012 semester. April 29th, 2011 is the deadline for returning students to submit the application. First year students entering Wake Forest in Fall 2011 should apply in the fall.
