Information Systems now uses the “IS-Requests Form” for recording requests for systems changes and enhancements. This form is an intermediate step in our new IT project management process. Later in 2007 @task, a collaborative scheduling and communication software tool, will further enhance our new approach to IT project management.
Please complete the Service Request Form by following these steps:
1. Download the form. |
2. Save it on your computer. |
3. Open the form using Adobe Acrobat 7.0. |
4. Save the form and rename it using the following convention:
Department (5 letters or less, lower case only)_username_date.
For example, Pat R. Smith in University Advancement would submit a request saved as: ua_smithpr_081507.pdf.

5. Complete the form. |
6. Submit the form as an attachment to is-requests@wfu.edu. |
