- University paid
- Conversion privilege
- Elimination Period - 6 months of continuous disability
- Monthly Benefit & 60% of monthly wage base, not to exceed
a benefit of $10,000
- Minimum benefit - $100
- 10% monthly annuity premium benefit
- Rehabilitation Service
- Social Security Disability Assistance
- Coverage not available if disability is caused, or contributed to, by:
- Intentionally self-inflicted injury;
- War;
- Pre-existing condition – accident, injury, that occurred one full year prior to eligible date
- Benefits continue until the following age or time limit;
At time of disability -
- If age less than 60 benefits continue to age 65
- If age 60 but less than 65 benefits continue for 4 1/2 years
- If age 65 but less than 68 1/2 benefits continue to age 70
- If age 68 1/2 or over benefits continue for 1 year
