If you would like to look up a copay for a particular drug,
go to Drug Information (on the left of your screen). Key in
the name of the drug, then click "Drug Copay" under
"more information". Click on the $ sign and this
will show you what your copay is.
The reimbursement form for using a drugstore outside of the
network, or if you have misplaced your card and had to pay
for your prescription, may also be found on the Catalyst Rx
website.
