North Carolina Licensure
Application Process
The following steps make up the application procedure for the North Carolina Teaching and School Counseling licenses. Applicants are responsible for the completion of their licensure application. No application will be forwarded to the North Carolina Department of Public Instruction until all documents have been submitted to the Licensure Office. When all documents have been received, the application will be mailed to the North Carolina Department of Public Instruction for approval and processing.

- Form A
- Praxis II Score Reports—for Elementary, School Counseling and Lateral Entry Students only
- Final Transcript(s) Official Graduate and Undergraduate Wake Forest transcripts will be requested by the Licensure Officer to expedite the processing of applications. The transcripts must include all course work at Wake Forest University and other institutions. The applicant is responsible to submit any undergraduate transcript from another institution to the Licensure Office.
- Processing Fee The processing fee is $55. Money Orders, Certified Checks or Personal checks should be made to the NCDPI.
- Form E (Optional) Form E is completed by applicants who have taught one-half time or more (fifteen hours per week).
- Form S Form to be completed by the Cooperating Teacher and Principal in LEA.
Not applicable to School Counseling students.
A letter will be mailed to you after your application has been mailed to the North Carolina Department of Public Instruction. The letter states the date your application was mailed to NCDPI. The processing of your application usually takes from two to four weeks. Any questions regarding the status of your license should be directed to NCDPI at 1-800-577-7994 or 1-919-807-3310.
