Creativity Symposium - Registration
New Registration Deadline: March 5, 2009 Registration fee: $235 REGISTRATION FORM: Click here to access the Registration Form. If paying by check, please make checks payable to Wake Forest University and mail with your completed registration form to Office of Entrepreneurship and Liberal Arts, Wake Forest University, PO Box 7229, Winston-Salem, NC 27109-7229, Attn: Aimee Mepham. To pay Registration Fee with a Credit Card, please call Aimee Mepham at 336-758-2657, Monday-Friday, 9am-4pm with your credit card information. Information for the registration form will be collected during this call. All symposium participants (including presenters and members of the WFU community) must complete a registration form by March 5, 2009. You can download the registration form above and email it to Aimee Mepham at mephamam@wfu.edu or mail it to the Office of Entrepreneurship and Liberal Arts, Wake Forest University, PO Box 7229, Winston-Salem, NC 27109-7229. Registration forms can also be hand delivered to Aimee Mepham in the Office of Entrepreneurship and Liberal Arts, 128 Manchester Hall (Reynolda Campus). Please indicate which days you will attend on the registration form (even if receiving free admission as WFU student/staff/faculty). To assure entry to all symposium events, we encourage all attendees to meet the MARCH 5th DEADLINE for registration. After that date, registration will take place on a first come, first served basis. As many events have limited seating, we will not be able to guarantee admission to all events after March 5th. Symposium Events are free to all members of the Wake Forest University Community and free to all undergraduate students from participating colleges and universities (meals not included). One-Day Fee: $125, Thursday or Friday Half-Day Fee: $55
Wednesday, March 18, Opening Keynote Address with David Bornstein: $20 Friday, March 20, Closing Keynote Address with Meredith Monk: $20 Full Symposium fee includes admission to the Meredith Monk Performance at 7:30pm on Thurs., March 19. Accommodations Blocks of rooms have been reserved for symposium participants at the Courtyard Marriott and the Residence Inn Marriott in Winston-Salem. See below for locations and prices. Commercial airlines serving the Piedmont Triad Airport include American Airlines, Continental, Delta, Northwest Airlines, United Airlines, and USAirways. Taxis and rental cars are available, and express shuttle service is operated by PART from the PTI Airport to each major city in the Piedmont Triad. The fare for PART is $2 for adults and transfers are free. You can visit www.visitwinstonsalem.com or the WFU Visitors' Center for more information on cabs, limousine services, shuttles and rates. Charlotte Douglas International Airport (CLT) is about an hour and twenty minutes away. Information on transportation to Winston-Salem from CLT can be found at the airport website. Parking If driving to campus, parking is on a first come, first served basis in Q Lot. From the Reynolda Rd. entrance to campus, you will be on Wake Forest Rd. Make the first left onto Allen Easley St. and the next right onto Aaron Ln. into Q Lot. If Q lot is full, overflow parking is available in First Assembly A lot, located across the street from campus off of Long drive. Exit Q lot from the further end away from Scales Fine Arts Center, and make a right onto Carroll Weathers Dr. Make the first left onto Wingate Rd, and cross the intersection at the traffic light. Wingate Rd. turns into Long Dr. Make the first left from Long Dr. into First Assembly A Lot, located across the street from the church.
