Confidentiality 

Designated Office
The Learning Assistance Center is responsible for housing the disability information which is disclosed by a student seeking accommodations. The Director of the LAC is responsible for maintaining this information in confidential locked files separate from the student's academic file.

Need to Know Basis
Disability information is shared with faculty and staff with the student's written permission on a limited "need to know" basis.  Common situations for this need include implementing testing accommodations or arranging accessible housing.

Disposal of Records
The confidential file is maintained for seven years after the student has graduated or left Wake Forest University. The file is then shredded.
