Student Athlete Information
First-year Student Athletes
All first-year Wake Forest University student athletes (freshman or tranfers) must submit a Pre-participation Health Form and a copy of their current health insurance card prior to arriving on campus. Once arriving on campus, pre-season physical examinations will be conducted at a date and time to be determined . Student athletes can download their form (below) and mail it along with their health insurance card (front and back).

Returning Student Athletes
All returning Wake Forest University student athletes must submit
an Annual Health History Form, Student Athlete Insurance Form and
a copy of their current health insurance card prior to arriving on
campus. These forms will be mailed out on July 1, 2005. Student athletes
can also download the forms (below) and mail it along with their health
insurance card copy (front and back).

Student Athlete Sports Medicine Survey Results
Final results of the first-annual Wake Forest University student athlete sports medicine survey are now available.
